The following information is required so that we may properly assess your financial position prior to filing your assignment in bankruptcy or consumer proposal. The information requested includes personal, family, employment, tax, and financial data. If you do not understand a question in the application or if you require assistance completing the application, please contact our office - we'll be pleased to assist you.

Instructions
- How To Fill Out The Application
Please answer every question. If a question does not apply to your situation, simply mark the box n/a. You must provide us with proper mailing addresses and account numbers for your creditors. This information can be obtained from your monthly statements or the telephone book.
- What to do When Application Completed
After you have completed the application, please return it to our office by mail, e-mail or fax it to us at (519) 633-0912. When we receive your application we will set up an appointment with you to sign the prescribed documents.
- Filing Fee
When we file your bankruptcy or proposal, we are required to pay a filing fee (effectively a tax payable to the Government). Therefore we cannot file your bankruptcy or proposal without a deposit.
- Payments to the Trustee
We offer free consultations and we are please to provide you with as many free consultaions as you require. However upon signing bankruptcy or proposal documents, we will require you to make your first payment to us. In most situations we will accept monthly payments which can be made by post dated cheques, pre-authorized payments ("PAP") or by making monthly payments in cash or by money order.
